Xeljanz Safety Concerns

The FDA required the manufacturer of Xeljanz, Pfizer, to conduct a randomized safety clinical trial when Xeljanz was first approved by the FDA. After reviewing their final trial results of Xeljanz, the FDA found that both doses of Xeljanz were associated with higher rates of heart attack, stroke, cancer, blood clots and death when compared to TNF blockers.

The FDA found that those taking Xeljanz were 1.33 times more likely than those taking TNF blockers to suffer cardviovascular death, nonfatal heart attack or nonfatal stroke. The FDA also found that those taking Xeljanz were 1.48 times more likely than those taking TNF blockers to develop malignancy, excluding nonmelanoma skin cancer.

Xeljanz is a type of drug known as a JAK inhibitor. The FDA has stated that other JAK inhibitors such as Olumiant and Rinvoq may have similar risks to Xeljanz since they share mechanisms of action with Xeljanz.

Xeljanz Warnings

The U.S. Food and Drug Administration (FDA) approved Xeljanz in November 2012 for treatment of rheumatoid arthritis, psoriatic arthritis and ulcerative colitis. Xeljanz XR was later approved in February 2016. Xeljanz XR is an extended release version which only needs to be taken once per day.

The FDA issued a drug safety communication about Xeljanz in February 2019. The communication said rheumatoid arthritis patients who take 10 mg of tofacitinib (the active ingredient in Xeljanz and Xeljanz XR) twice per day face an increased risk of death and blood clots in the lungs.

The FDA later approved a Boxed Warning for Xeljanz in July 2019. The warning concerned an increased risk of blood clots and death in ulcerative colitis patients who take 10 mg of tofacitinib twice per day.

The FDA then announced in September 2021 that it was requiring a Boxed Warning for the labels of Olumiant and Rinvoq, warning of serious heart-related events, blood clots, cancer and death.
