PADCEV, Stevens-Johnson Syndrome And Toxic Epidermal Necrolysis

The United States Food and Drug Administration (FDA), on July 9, 2021, approved PADCEV for some adult patients with urothelial cancer. However, in approving the medication for some patients, the FDA mandated that its strongest warning, a “black box” warning, be added to the drug’s label.

This warning notes that PADCEV can cause “severe and fatal” skin reactions, including SJS and TEN. The label also notes that skin reactions occurred in 55% of the 680 patients which clinical trials saw treated with PADCEV.

The “black box” warning was prompted by research which identified eight cases of SJS or TEN caused by PADCEV from December 2019 to October 2020. Four cases resulted in death and four cases led to hospital burn unit admission.

The drug had seen accelerated FDA approval for some urothelial cancer patients in December 2019. This means that urothelial cancer patients have been taking a drug which can cause horrifying skin reactions for over a year and a half without having been warned about it.

California law holds drug companies responsible for injuries caused by their drugs when they fail to warn about the potential for the injuries when they know or should know about the potential. The term Toxic Epidermal Necrolysis refers to the same skin reaction as Stevens-Johnson syndrome. It refers to a skin reaction which involves over 30 percent of the skin. 30 to 40 percent of TEN patients die as a result of the reaction.
